Take the HLQAT Survey

The tool is designed as a gap assessment which, when coupled with the RLI toolkit, will provide a strategic roadmap to quality excellence.

The HLQAT consists of two distinct surveys for individuals working in two different groups of hospital leadership positions, Senior Leadership and Clinical Management. Each questionnaire includes just over 100 questions that examine structures, processes, and leadership activities demonstrated to be associated with high performance in clinical quality in a hospital setting. It can be completed in about 30 minutes.

 
 
 
 
  • HLQAT Update

    The HLQAT Team is pleased to announce that HLQAT.org will remain open until March 31, 2011, for hospitals seeking to take their re-measurement survey.

    In order to take the HLQAT survey again, simply enter the Hospital Code that you used the first time you took the survey, but with the suffix "_2" added to the end. For example, if your initial code was H01111, your remeasurement code will be H01111_2.

    Be advised that recently completed surveys will not be swept into the "View Reports" section of HLQAT.org immediately, so don't worry if you're not able to view your hospital-specific reports for a week or two.
